Diabetes Care Management Program

The Diabetes Care Program is a patient care program for people with diabetes. It is a major resource for primary care physicians, diabetic patients, and the community as a whole. The program includes information, assistance and ongoing support to assist participants in the program to reach their diabetes self-management goals and improve their confidence in living with and managing diabetes.

Program Fee

There are fees for the Diabetes Self-Management Education appointments and group classes and Medical Nutrition Therapy services. Most insurance plans will cover this service. Financial assistance is available.
